Category 1 and 2 lesions (simple cysts and mildly complicated cysts, respectively) are straightforward to diagnose and are benign, not requiring additional follow-up. Category 4 lesions (typically cystic neoplasms) are also easily diagnosed as neoplastic and require surgery.
